Good Faith Estimate Notice
You have the right to receive a Good Faith Estimate explaining how much your medical care will cost. Under federal law, health care providers must give patients who do not have insurance or who are not using insurance an estimate of expected charges when care is scheduled at least 3 business days in advance or when an estimate is requested. A Good Faith Estimate may include expected charges for items and services provided by a single provider or facility. It may not include separately scheduled services, care from another provider or facility, or additional services that could not reasonably be anticipated. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be able to dispute the bill. Keep a copy of your estimate and bill. For questions about your rights, contact the No Surprises Help Desk at 1-800-985-3059 or visit cms.gov/medical-bill-rights. For an estimate from Boerne Foot & Ankle, please contact the office before scheduling or when requesting services. This notice is informational and does not replace an individualized estimate.
